This forms you can order by phone in the USA: 800-870-3676 by leaving a voice message asking for "Fiancée Visa Kit" and leaving also your name and address so it will be mailed to you. The "Fiancée Visa Kit" includes following: (1) I-129F, (2) G-325A, (1) I-134 and instructions; plus some carbon copies of G-325A. It will take approximately from 2 to 4 weeks to receive everything. You will submit original papers to INS later, except I-134 only as a copy. If you are visiting your lady you should take the following papers with you:
